Wellbeing Coaching Agreement

What can I expect from coaching?
Wellbeing coaches can help you increase wellbeing in many areas, including, but not limited to, managing stress, physical activity, healthy eating habits, increasing energy, and increasing mood. Your coach will provide a space that is non-judgmental and safe for your coaching sessions. Your coach is not here to “tell you what to do,” but to help guide you to develop a vision for your wellbeing and utilize your strengths and motivation to meet your goals. Your coach will also help you set gradual, manageable steps to meet those goals.

What will not occur during coaching?
Your coach cannot provide counseling or therapy for diagnosed mental health disorders. If you require counseling for a mental health disorder, you may make an appointment with one of our mental health counselors at 509-359-2366. During your coaching sessions, your coach may determine that it is more appropriate for you to speak with a counselor dependent on the issue. In that case, you will be referred to the appropriate resources.

Your coach cannot provide medical advice. Although not required, it would be helpful for you to disclose any medical conditions to your coach during the pre-assessment period or during your first coaching session. This will allow your coach to determine if you will require medical supervision throughout the coaching process, or if a medical professional would better help you achieve your goals.

Rights of the Student
If you feel that your needs are not being met by your wellbeing coach, or if you feel that the relationship between you and your coach is not a good fit, you may request to switch coaches or terminate your coaching agreement.

Session Limits
There are no session limits for wellbeing coaching. Most coaching sessions can be completed within one quarter. During your first session, you and your coach will review your overall goals to determine how often you may need to meet to achieve those goals.

Sessions are scheduled year-round. Coaching will be available during the Summer quarter for students who are currently enrolled or who provide proof of return for the following Fall quarter. Sessions will not be available during university holidays or quarter breaks.

Wellbeing coaching is only provided to currently enrolled EWU students. Coaching will terminate upon graduation from the university or if you discontinue enrollment. If you are interested in continuing coaching at that time, your coach may refer you to local resources.

Fees
Wellbeing coaching sessions are included as part of your comprehensive student health fee. There are no additional fees required to utilize these services.

Appointments
Wellbeing coaches are available during scheduled hours. Hours may vary by quarter based on availability of the coaches. Your coach will discuss any changes of hours with you during your sessions or prior to the upcoming quarter, if your sessions will continue.

All coaching sessions are by appointment only through EAB Navigate. You can access appointment scheduling through a link on our webpage.

If you are unable to make your appointment, please provide a minimum of 24 hours notice via phone or e-mail directly to your coach. Failure to provide notice of a cancelled appointment may result in your being dismissed from the wellbeing coaching program.

Sessions
Prior to your first session, you will be required to complete a wellbeing assessment. The assessment will cover a broad range of health and wellbeing issues to allow your coach to best understand your needs.

Your initial session will last 60-75 minutes. Your wellbeing assessment will be discussed during your initial session.

Subsequent sessions will last approximately 30-40 minutes, but no longer than 45 minutes.

Privacy
Coaching sessions are private. Coaching records are protected by the Family Education Rights and Privacy Act (FERPA) and release of those records will be governed by WAC Chapter 172-191, Student Education Records.

There are exceptions to privacy, which include disclosure that you may be of harm to yourself or others, that an incidence of sexual misconduct has occurred, or that a commission of a crime has occurred. These disclosures will be reported to the appropriate campus resources.

Liability Waiver
In consideration of my being allowed to receive coaching services from a CW and/or CERTIFIED WELLCOACHES Member, and, in that process, to be coached in fitness, nutrition, weight management, stress management, wellness, personal coaching, and/ or health risk management, I do hereby waive, release, and forever discharge Wellcoaches Corporation and its officers, agents, independent contractors, employees, representatives, executors, and all others from any and all responsibility or liability for injuries or damages resulting from my participation in any activities or my use of fitness equipment or any other equipment or machinery arising out of my participation in any activities under such coaching.

I do also hereby release all of those mentioned and any others acting upon their behalf from any responsibility or liability for any injury or damage to myself, including those caused by the negligent act or omission of any of those mentioned or others acting on their behalf or in any way arising out of or connected with my participation in any activities of coaching.

I understand that as a part of the coaching relationship I may be coached to, or it may be suggested that I, participate in fitness activities, e.g., exercise, aerobic training, strength training, flexibility training, etc., that could be potentially hazardous. I also understand that such activities involve risks of injury and even death, and that I am voluntarily participating in these activities and using equipment and machinery with knowledge of the dangers involved. I hereby agree to expressly assume and accept any and all risks of injury or death.

I further understand that my CW and/or CERTIFIED WELLCOACHES Member is an independent contractor and not an agent of Wellcoaches Corporation.

I do hereby further acknowledge that I have either had a physical examination and have been given a physician`s permission to participate or that I have decided to participate in activity and or use of equipment and machinery without the approval of my physician and do hereby assume all responsibility and risks of injury or death from such participation and activities.

I understand that I can revoke this authorization at any time and that will result in ending my coaching engagement. I agree to the liability waiver terms and conditions listed above for participation in the Wellcoaches Corporation coaching program.
