If the academic program is already banked or discontinued, do I have to complete a template?

Yes, all programs identified on the published list must complete a template. To make the process more efficient for those programs, a question at the beginning of the template will prompt the user to indicate if the program has been banked or discontinued, in which case, the template can be submitted with no additional information inputted.
