Request a Waiver

What is a Waiver?

A waiver is a legally binding contract, in which the participant in an activity agrees to not hold the university responsible or "liable" for any injuries that the participant might receive as a result of participating in the university's programs.

How to Request a Waiver

To request a waiver, fill out the form below, completing all the questions to the best of your knowledge. Make sure to fill out the form at least 7 business days before your event, as to allow for time to get your waiver request approved.

  • I further agree that if I choose to drive my own motor vehicle for transportation to, during or from the event site, I am responsible for my own acts and for the safety and security of my own vehicle. I accept full responsibility for the liability of myself and my passengers, and I understand that if I am in a vehicle, EWU and its personnel are not in any way responsible for the safety of such transportation and that the State of Washington insurance does not cover any damage or injury suffered in the course of traveling in such a vehicle.
