\n Complete all applicable sections, but do not sign the document before your\n appointment unless you know that it requires an acknowledgment of a signature\n already made. When in doubt, leave it unsigned.\n <\/div>\n <\/div>\n\n

\n Government-issued identification for every signer<\/strong>
\n Examples include a passport, driver\u2019s license, or state identification card.\n Identification must be current or expired by no more than three years.\n <\/div>\n <\/div>\n\n
