To fill out the form online<\/strong><\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Go to:&nbsp;<a href=\"http:\/\/shopeaglerec.ewu.edu\/\">shopeaglerec.ewu.edu<\/a><\/li>\n\n\n\n<li>Click &#8216;LogIn&#8217; in upper-right corner.<\/li>\n\n\n\n<li>If a student, Login via NetID<\/li>\n\n\n\n<li>Read\/sign\/submit the electronic waiver.<\/li>\n\n\n\n<li><em>*Please Note* waivers are required once per year and are active for 365 days.<\/em><\/li>\n<\/ul>\n\n\n\n<p><strong>If you are a student under the age of 18, the following steps are required:<\/strong><\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Download\/print the Waiver for Minors and have parent\/legal guardian sign;<\/li>\n\n\n\n<li>Call the URC Front Desk (359-4019) or the Fitness Center Front Desk (359-2639) to give notification that you have the waiver ready to submit;<\/li>\n\n\n\n<li>URC Staff will &#8220;create&#8221; your membership manually;<\/li>\n\n\n\n<li>Once you have a membership, you will be able to schedule workouts.<\/li>\n\n\n\n<li>When you come to your first workout, BRING YOUR WAIVER WITH YOU (we can&#8217;t let you in without one on and\/or Catalyst Building) are eligible to use the U-Fit Center, located in the&nbsp;Spokane Center for Clinical Research and Simulation (SCRS) building.
